This is a reflective essay based on my experiences whilst on my five week surgical placement on a urological ward in a local hospital. The aim of this essay is to explore the use of communication and interpersonal skills in clinical practice. I have chosen this particular incident as I spent a considerable amount of time communicating verbally and non verbally with this particular patient. To protect the identity and maintain confidentiality of the patient I have chosen to discuss they shall be known as 'John', this is accordance with the NMC code of professional conduct (NMC 2008). To assist me in the process of reflection I am using Gibbs (1988) reflective model. This cycle consists of a description of the event, feelings towards what happened, an evaluation, an analysis, a conclusion, and an action plan.

Chronic Heart Failure (CHF) Disease

Chronic heart failure (CHF) is increasing at an alarming rate as the most common form of cardiovascular disease. One cause for this increasing trend is attributable to advances in medical management of coronary heart disease and a ageing population. Patients with CHF present with signs of fatigue and shortness of breath (SOB) during, depending on the severity of the disease, activity and/or rest. The decreased exercise tolerance presented by CHF patients has been attributed to cardiac limitations (reduced left ventricular function) as well as a decrease in peripheral muscle endurance and strength. Reduced peripheral muscle strength and endurance is explained to be caused by decreased muscle perfusion and changes in skeletal muscle morphology and oxidation characteristics (Volaklis 2005).

Bias in Epidemiologic Research

For the following study descriptions, describe potential bias (es) most likely to be present. Indicate how differential bias would affect the measure of association (would it be underestimated, overestimated or unchanged):
1. A cohort study was designed to look at the association of pesticides and the occurrence of childhood leukemia. Children living in rural areas were followed and leukemia rates were compared between those children living in areas with high and low pesticide exposure. Pesticide exposure was considered high if it was over 20 “units” and low if it was under 20 “units”.

About Asthma

As proved by all reports and statistics worldwide, asthma has increased markedly in recent decades and is continuing to increase, even in countries that have been relatively unaffected by the disease in the past. Although genetic factors are important in the manifestation of asthma, the rapid increase in the prevalence of this disorder cannot be related to genetic factors alone. Changes in the social and economic environments to more industrial and modern also need to be taken into account. According to WHO estimates, 300 million people suffer from asthma and 255 000 people died of asthma in 2005 (WHO, Online). It is the most common chronic disease among children.

Aging and Disabilities

Australian population is ageing. It has been widely disseminated that the over-60s now outnumber the under 16s and this trend is set to continue. (MacKinlay, 2009) Increased average wealth, advances in medical science, better living conditions and the first 'baby boomers' reaching retirement all contribute to the growth in numbers of older people. Living longer is a fact to celebrate. However, people are not necessarily living these extra years in good health. Older men can now expect to live 6.9 years with a disability and older women 9.2 years. (Brown, 2004)

What is Fat Tax?

Allowing our government to regulate our society's weight by taxation is an absolute "No!" While taxing "unhealthy" products is constitutional, I do not believe it is the right step in preventing obesity. People are personally responsible for their weight, not the products. How would the government decide what items to tax? Practically any product could make you overweight if you ate it out of moderation. Allowing such a "fat tax" would only punish businesses.

Healthcare Roles in Communications

Health communication is perhaps one of the most influential and controversial aspects of the health care system today. A multitude of influences can affect the ultimate success or failure of collaborative communication between patient and caregiver, which may result in mutual frustration and a lack of effectiveness towards the healing process. Though the remedy of it is not yet a top priority for organizational culture improvements, low health literacy is one of the biggest problems affecting patients and caregivers in the United States. Enlargement of sebaceous glands and an increase in sebum production occur with increased androgen (DHEA-S) production at adrenarche. The microcomedo may enlarge to form an open comedo (blackhead) or closed comedo (whitehead).
